Pride Dental Group ("we," "us," "our") keeps records of the care we provide you. We are required by law to protect the privacy of your health information, to give you this notice explaining our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
We use your health information to provide dental care: your dentist, hygienist and assistants review your chart, x-rays, medical history and medications so treatment is safe. We share it with others involved in your care, such as a specialist we refer you to, a dental laboratory making a crown for you, or your physician when a medical condition affects your treatment.
We share what your insurer needs to confirm coverage and pay a claim: your name, date of birth, member details, the procedures performed and supporting x-rays or notes when they ask for them.
We use your information to schedule appointments, review the quality of our care, train staff, and handle billing questions.
We may call, text or email you about appointments, forms to complete before a visit, treatment you are due for, and reminders that it is time for a checkup. Our text messages and emails name the practice and carry a link or a short message only; we do not put details of your treatment in them. You can tell us to stop at any time by replying STOP to a text or by calling the office.
We may use or share your information without your permission when required by law, including: public health reporting; reporting suspected abuse, neglect or domestic violence; health oversight activities such as audits and inspections; responding to a court order, subpoena or lawful request from law enforcement; work with a coroner, medical examiner or funeral director; approved research; and to avoid a serious threat to health or safety.
We will not sell your information, use it for marketing in exchange for payment, or share psychotherapy notes without your written authorization. If you give permission and later change your mind, you can revoke it in writing at any time, and we will stop, except where we have already acted on it.
We are required by law to keep your health information private, to follow the terms of this notice, and to notify you if a breach occurs that may have compromised your information. We can change this notice, and the change will apply to all the information we hold. The current version is always posted on this page, and a copy is available at the front desk.
Access to your records is limited to the people who need it to care for you or to run the practice. Our systems require a sign-in, and every time a record is opened it is logged.
